If I am a new Patient what should I bring ?
We ask all of our new patients to arrive to their appointment ten minutes early to allow time to complete our new patient packet. Please have your current insurance cards available for us to copy, as well as a picture id. We photocopy picture Id‘s for security and privacy purposes. Please bring a list of all current medications so we are able to copy and record this list into our files. If you are a new patient to our office and are transferring from another Physician we ask that you allow 15 business days for their office to transfer records to our office. We can obtain records one of two ways from your previous physician, you can call your previous physicians office and request records be sent to our office via fax or mail, or you can sign a release with our medical records department and we can fax your request to your previous physician. Please remember that if there is a co-payment with your insurance company we will collect this amount at the time of service.

Do you carry my insurance ?
We accept others and are always looking for new panels to join so be sure to
ask about your insurance if it is not listed below. We will be happy to call
them for you prior to your treatment.

What is the office phone policy?
Our phone hours begin at 8:30 am, and end at 4:30 pm, Monday - Thursday. Because we do not believe in an automated phone system, our staff is trained and qualified to handle all phone calls as they come in. Calls are then transferred to the appropriate staff member who can better assist your call. If a message needs to be taken, a detailed hand written message will be delivered directly to the person who will return your call. All patient call backs are done in between patients, as the schedule permits, at the end of the day, or in rare cases the following business day.
Are Physicians available after hours for emergency purposes?
For medical emergencies CALL 911. For non-emergency purposes we do have a voice-mail that allows patients to leave detailed messages during non working hours. You are encouraged to use our Patient Portal.
What should I do to cancel my appointment?
If you have to cancel an appointment, please call us at 682-999-8446, 24 hours prior to your appointment. Our front office staff will also call you the morning/afternoon of your missed appointment in attempt to re-schedule according to your convenience. We strive to go above and beyond for our patients, and in turn ask for your courtesy and cooperation with our policies. Please see our Appointment Policy.
Is my weight loss/visit covered by my insurance ?
It is very important that you send us the copy of (front and back) insurance cards, social security and driver's license before making an appointment. This allows us to check if we accept your insurance and if your benefits are covered. We then call you explaining your benefits what will be covered and schedule for an appointment. Please see our Insurance and Appointment Policy in place.
Food and compound injections such as Lipo-B are not covered.
Body Composition Analysis ($10) is not covered and it is an out of pocket expense.
Please note that office visits and any medical treatment will be covered as per your benefits.

Will I get a call for my lab results ?
We only call if there is a need to discuss the results right away as they arrive, otherwise it will be discussed on your next appointment or will
be made available through our Patient's Portal once the Physician has looked at it and signed it. We do this on a regular basis and is on our task list.
How do I get access to Patient Portal ?
Registration link would be sent to you via email to be completed before you are here for your first appointment.
In the event, you are not able to do so, please plan to be at the office at-least 30-45 minutes prior to your appointment.
Please provide the medical insurance card and your driver's license when you are at the office.
You will receive the access via the email to get access to patient's portal.
In the event if you are not able to activate the account, we will provide you the username and password.

Enter Username and Password provided to you and enter your Date of Birth to activate your account.
